CHS PTSA STAFF

COMMITMENT TO EXCELLENCE AWARD

Each year, the CHS PTSA recognizes and awards to a dedicated staff member who  demonstrates a committed and caring attitude toward students and fellow staff members. The winner will receive a $500.00 monetary award and their nameplate will be added to the Commitment to Excellence eternity plaque displayed in the CHS Main Office.  Nomination forms are available here and in the CHS Main Office.

Requirements:
1. Nominees must be current CHS staff members, teaching or non-teaching.

2. Nominations will be accepted from current 2025-2026 CHS staff, students, parents and CHS community members (for example: business, non-profit, government entity, etc.).

3. Only one nomination will be accepted per person.
